Quality of life and maintaining the highest level of function is what we strive for with all of our patients. Our long term residents appreciate the relationships, care and focused attention that the rehabilitation staff give them. We focus on safety, education, function, strength, and balance with the patient’s and caregiver’s goals in mind.

We focus on creating patient specific protocols and managing the rehabilitation aspect of our patient’s progress while working with the team to develop a cohesive discharge plan. Through our resources through the various disciplines we educate patients and caregivers on progress. Many patients and family members are relieved by the amount and degree of involvement that therapy has in the day to day care and monitoring of patients and their progress.

Our network and experience allows us to anticipate changes prior to these being enacted. In addition, we are able to keep our Facility Partners up to date on all ongoing clinical, regulatory, administrative, and infectious protocols.

In addition, along with physician partners we are able to support and continue to co-market through lectures, networking, and presence.

The Patient Driven Payment Model has allowed us to now get appropriate reimbursement for the acuity of patients that we care for in our facilities and truly utilize therapy and provide services as deemed necessary by the therapy teams. Utilizing the onsite MDS team along with our own MDS consultants our facility partners have optimized and captured diagnoses that significantly impacted reimbursement. We have also acted as a liaison and leader in establishing and running weekly team PDPM meetings for our facility partners to maintain this level of efficiency.
